Roux-en-Y · Gold Standard

Gastric Bypass

The gold standard in bariatric surgery for 50+ years. Creates a small stomach pouch and reroutes the intestine — producing powerful restriction and hormonal changes. Best for patients with severe GERD or Type 2 diabetes.

Sleeve Gastrectomy · Most Popular

Gastric Sleeve

The most commonly performed bariatric surgery in the U.S. (61% of all procedures). Removes 70–80% of the stomach, creating a sleeve-shaped tube. Simpler than bypass with excellent results and a faster recovery.

BPD-DS · Most Powerful

Duodenal Switch

The most effective bariatric procedure available. Combines a gastric sleeve with an extensive intestinal bypass for the highest weight loss and diabetes remission rates of any surgery. Best for BMI 55+ or severe metabolic disease.

SADI-S anatomy
ASMBS Endorsed 2020
Single Anastomosis · Newest Procedure

SADI-S

The most innovative bariatric procedure — delivers results nearly identical to the traditional Duodenal Switch with just one intestinal connection instead of two, resulting in lower complication risk and fewer nutritional side effects. Also excellent for sleeve revision.

Lap-Band · Adjustable

Adjustable Gastric Band

The only reversible and adjustable bariatric procedure — a silicone band placed around the upper stomach. Outpatient surgery with minimal recovery time. Now accounts for only ~3% of U.S. procedures. We also support existing band patients from other surgeons.

Revision & Conversion Surgery

Bariatric Revisions

If a previous bariatric procedure hasn't delivered results or has caused complications, revision surgery can get you back on track. We accept patients from other surgeons and offer the full range of revision and conversion options — including Band to Sleeve, Sleeve to Bypass, and Sleeve to SADI-S.

Intragastric Balloon

A soft silicone balloon placed inside the stomach endoscopically — no surgery, no incisions, no hospital stay. Helps patients eat less by taking up stomach space. Available for 6 months. Note: Our surgeons are trained on both systems but do not currently offer balloon treatment at Better Weigh Center.

How Do You Know Which Procedure Is Right for You?

There is no universal "best" bariatric procedure — the right choice depends on your unique combination of health factors. Here are the key considerations your surgeon will evaluate:

Your BMI

BMI is the starting point. Sleeve is most effective at BMI 35–50; bypass excels from 35–55+; SADI-S and DS are preferred at BMI 50+. But BMI is just one factor — health conditions matter equally.

Acid Reflux / GERD

If you have significant GERD, gastric bypass is strongly preferred — it resolves reflux in 90%+ of cases. The sleeve can worsen reflux in some patients. This single factor can shift the recommendation significantly.

Type 2 Diabetes Severity

For severe or medication-resistant diabetes, the DS or SADI-S (both ~90–95% remission) dramatically outperform the sleeve (~60%). The metabolic benefit often happens before significant weight is even lost.

Supplement Commitment

Procedures with intestinal bypass (bypass, SADI-S, DS) require lifelong daily vitamin supplementation. This isn't optional — deficiencies can be serious. Your willingness to commit to this long-term affects which procedures are appropriate.

Prior Abdominal Surgery

Previous abdominal surgeries create scar tissue that can affect which procedures are feasible. Our surgeons evaluate your complete surgical history to determine what's safe and practical for you.

Future Surgical Options

The gastric sleeve is the only procedure that can be converted to multiple different operations (bypass, SADI-S, DS) if more weight loss is needed later. If keeping options open matters to you, this is worth discussing.

The honest answer: you need a personalized evaluation.

Common questions

Frequently Asked Questions

Which procedure has the best results?
In terms of raw weight loss, the Duodenal Switch and SADI-S deliver the most weight loss (80–90% of excess weight). But "best results" depends on your goals — if acid reflux is a major issue, gastric bypass is the clear winner. If you want a strong outcome with lower nutritional risk than the DS, SADI-S is excellent. Our surgeons will tell you honestly which procedure gives you the best chance at your specific goals.
Does insurance cover bariatric surgery?
Many insurance plans cover bariatric surgery, but coverage varies significantly by plan. Most require a BMI of 40+ or BMI 35+ with a qualifying health condition. Some also require a medically supervised diet period before approval. Our team will verify your specific benefits and guide you through the pre-authorization process — we do this every day and know how to navigate it effectively.
How long is the recovery after bariatric surgery?
Recovery varies by procedure and individual. For the sleeve and bypass, most patients return to light activity within 1–2 weeks and full activity by 4–6 weeks. The Duodenal Switch typically requires a slightly longer recovery. All procedures at Better Weigh Center are performed laparoscopically or robotically, which significantly reduces recovery time compared to open surgery.
Can I still have bariatric surgery if I've had it before?
Yes — revision surgery is often an excellent option for patients who have experienced weight regain, complications, or insufficient results from a prior procedure. We accept patients who had their original surgery at other facilities. Revision surgery is more complex than primary surgery, so experience matters — our surgeons perform revisions regularly and have the expertise to handle these cases safely.
What's the difference between robotic and laparoscopic surgery?
Both are minimally invasive approaches that use small incisions rather than a large open incision. Laparoscopic surgery uses traditional long instruments; robotic surgery uses a robot-assisted system that gives the surgeon enhanced precision and a 3D view. Dr. Jegan Gopal performed the first robotic bariatric surgery in Corpus Christi. Both approaches produce excellent outcomes with faster recovery than open surgery.
Will I gain the weight back after surgery?
Some weight regain over time is common with all procedures, especially without consistent follow-up and lifestyle habits. The data shows that 70%+ of bariatric surgery patients maintain significant long-term weight loss — far better than the less than 5% success rate of non-surgical methods. The sleeve tends to have slightly higher regain rates than bypass at 5–10 years. Consistent follow-up with our program is the single most important factor in long-term success.
